This form is for ERT members who have participated on an ERT mission and who wish to recertify without taking the formal training again. 

ERT members who have not participated on an ERT mission and wish to recertify must go through the formal training.

After the submission of this form, you will receive a new training manual, a copy of an open book test, a background check permission form and a return envelope.  Return the completed test and background check permission form along with a check for $25 in the return envelope.  In addition, forward a digital photo of yourself to aehileman@umcswtx.org.  After the background check has been completed, a badge and T-shirt will be forwarded  to you.  Please contact us if you have questions.
